For adults and teens 13 years and older

- Unlimited monthly visits for each individual within the family plan(excludes labs)
- One time registration fee of $45 per adult and $25 per child. Monthly fee of $25/adult&teen and $15/child will begin the following month.
- Basic labs are included in the registration fee for individuals 18 years and older.
