Who We Are

 
 Grace Family & Urgent Care Clinic is a direct primary care and fee for service clinic. Patients who sign up for direct primary care pay a low monthly fee for direct access to the provider.  They have access to unlimited text, call, email, or video communication with their provider to get the hlep they need.   Under the fee for service plan, patients do not pay monthly; they pay for the service(s) utilized.  The advantages of the direct primary care plan include: unlimited access to the provider, continuity of care, comprehensive healthcare, cost savings (great for patients with conditions/circumstances that require more frequent follow up and/or larger families that are more likely to see providers), and ease of mind (knowing that you can access your provider with any type of question or concern).  

The mission of Grace Family & Urgent Care Clinic is to bring honor to the name of Jesus Christ by providing affordable healthcare that capitalizes on integrative medicine.

Our Providers

Grace Family & Urgent Care Clinic is staffed by board certified nurse practitioners.  Nurse practitioners are clinicians who are authorized to prescribe medications and provide comprehensive care for patients. Because nurse practitioners are first trained to be nurses, they are especially skilled at  listening and addressing patient specific needs and concerns.

Dr.Oluchi Ujoatu DNP, FNP-C, RN

Dr.Oluchi Ujoatu is a board certified nurse practitioner who obtained her doctorate degree in  nursing. She was a nurse for seven years before becoming a family nurse practitioner. She has been practicing as a family nurse practitioner for 11 years. She has practiced in family care clinics, urgent care clinics, and well as in emergency care clinics.  Her favorite part of being a family nurse practitioner is being able to connect with patients.
