provides primarcy care services as well as urgent care services.

insurance provider. This model normally entails a monthly membership fee.

We do not take insurance. We do, however, offer wholesale, affordable pricing
on items such as labs and diagnostics studies. Our prices are often cheaper
than what your coinsurance payment portion would be.
We provide memberships at the price of:
■ $20/month/child (7-13 years old) and
■ $59/adolescent or adult (14 years or older).
■ Family discount of $10/child (7-13 years old) and $49/adolescent or
adult (14 years or older)
Absolutely. Our services are utilized but both uninsured and insured persons.
We are able to make referrals just like other primary care clinics can.
Can I use my HSA or FSA to pay for member services?
Your monthly membership fees covers:
■ An unlimited amount of in person or virtual care visits
■ Chronic care visits,i.e. Diabetes, high blood pressure, high cholesterol,
thyroid disorders
■ Medication refills (excluding Narcotics)
■ Non emergent procedures such as:
● Simple sutures
● Infection and debridement of simple wounds
● Ear wax removal procedure
● Foreign body removal
■ Physicals such as sports physical, school physical, DOT exams
Yes. Grace Family & Urgent Care Clinic only provides primary and urgent
care services. We are not specialists and do not provide emergency care
services.
No. Concierge clinics normally charge high prices and bill insurance. We
offer affordable pricing and do not bill insurance.
To understand why, you must understand our current health care system.
Health clinics that accept insurance have high overhead costs that are associated with hiring more staff to ensure services are being reimbursed, i.e.
hiring billers, coders, etc. To keep up with these insurance related expense,
clinicians have to see high volumes of patients to remain profitable.
However, through charging low monthly fees, direct care clinicians circumvent
this. Clinicians under the membership based direct care model are able to
have lower patient volumes (due to lower overhead costs) and are therefore
able to spend quality time with patients, teaching and providing the necessary
tools to keep patients healthy
The direct primary care (DPC) clinic model emphasizes wellness. Although
you may only get sick once a year, in your membership is built in wellness
visits (which includes weight management).

Yes. You can cancel your membership at any time. If you cancel your
membership in the beginning of the month, your membership would be good
throughout the month but would cancel at the end of the month.
No, we do no accept walk-ins. Our members have unlimited access to us, so
accepting walk-ins would take away time from our members.
